Dual Displays lose settings

I'm running an iMac 5K (Yosemite 10.10.4) as my main monitor, with at Toshiba TV connected via HDMI to a Thunderbolt dock, which in turn is connected to the iMac via Thunderbolt (of course).

Periodically the monitors lose the settings I have for them.

1) Non-Mirrored

2) Certain height for the second display

3) Displays have separate spaces unchecked in Mission Control

Also when I turn on the second monitor, the iMac screen will black out twice as the TV turns on. In addition periodically, the second display will black out itself for an instant for no apparent reason.

Any suggestions?

Further info on this intermittent problem: I find that when it happens the iMac 5K comes up with the resolution labeled Toshiba TV, which is my 2nd monitor.

I'm using a Thunderbolt dock to receive the HDMI from the 2nd monitor, and feed it into the iMac with Thunderbolt...

When an app is open, position its window on the monitor you want it to always be on. Then R. click the mouse on that app's icon in the Dock... a menu will pop up, and under the "Options", will be "All Desktop". Or "This Desktop" That will lock that app to a specific screen (if that screen is connected).
